Key Takeaways
- The evidence is small and mixed. Small trials found less pain; the biggest, in 80 knee replacement patients, found none.
- Topicals work best close to the skin. Think hands, knees, elbows, jaw and feet, not hips or whole-body pain.
- Check the COA and the strength per gram, not just the milligrams printed on the jar.
- A "pain relief" cream with a Drug Facts panel gets its effect from menthol, methyl salicylate or lidocaine, not CBD.
- Full-spectrum topicals face a new federal THC cap of 0.4 mg per container starting December 11, 2026.
Does CBD cream for pain work? Sometimes. Small trials found less pain in spots close to the skin, like hands, knees, jaw and feet. The largest trial found no benefit. And menthol or arnica in the same jar may do part of the work. It's a local comfort tool, not a treatment for disease.
That's not what most CBD brands will tell you. It's just the honest answer.
In this guide, you'll get:
- What 6 studies found, including the 2 that flopped
- A body-area chart showing where a cream can (and can't) reach

Does CBD Cream Work for Pain? What the Research Shows
Short answer: sometimes, a little, in some spots. Only a handful of human trials exist, and they split down the middle.
Most pages you'll read only cite the wins. Here's the full scoreboard, misses included.
How Topical CBD Acts on Your Skin
Your skin has its own cannabinoid receptors. They're part of the endocannabinoid system, a signaling network involved in pain and inflammation. When you rub in topical CBD, it acts on receptors right where you apply it, including TRPV1, which handles heat and pain signals.
Very little CBD from a cream reaches your bloodstream, so any effect stays local. How much gets through the skin at all is still unknown. The FDA lists it among its open questions.
"Whether these products deliver CBD below the skin isn't clear." (Arthritis Foundation)
What 6 Studies Found (Including 2 That Flopped)
Here are six controlled studies, including two null trials most CBD pages skip.
| Study | Who took part | What they used | What happened |
|---|---|---|---|
| Xu et al., 2020 | 29 adults with peripheral neuropathy in the legs and feet | CBD oil on the skin for 4 weeks vs. placebo | Less intense, sharp, cold and itchy pain |
| Nitecka-Buchta et al., 2019 | 60 adults with jaw muscle pain (TMD) | CBD on the jaw muscle twice a day for 14 days vs. placebo | Pain scores fell about 70%, vs. about 10% on placebo |
| Heineman et al., 2022 | 18 adults with arthritis at the base of the thumb | CBD cream twice a day for 2 weeks vs. shea butter alone | Better pain and hand-function scores, no side effects |
| Haffar et al., 2022 | 80 adults after knee replacement | CBD on the knee after surgery, plus usual pain care, vs. placebo | No less pain or opioid use than placebo |
| 2025 pilot trial | 28 young adults after a leg workout | CBD cream on the thighs vs. placebo cream or nothing | No less soreness, no change in performance |
| Hammell et al., 2016 | Rats with arthritis | CBD gel on the skin | Less joint swelling and pain behavior (animal study) |
See the pattern? The wins came in small groups with pain in one shallow spot: feet, jaw, thumb. The misses tested a new knee joint and whole thigh muscles.
Here's the truth: nobody in these trials used CBD for more than four weeks, and the thumb study had just 18 people. Promising, yes. Proven, no.
The Menthol Question Nobody Asks
Many CBD creams also contain menthol, camphor or capsaicin. Those are approved over-the-counter pain relief ingredients in their own right.
The Arthritis Foundation notes this makes it hard to tell which ingredient did the work. So check what else is in the jar:
- Menthol or camphor: a cooling feel within seconds
- Capsaicin: a warming feel from chili pepper extract
- Arnica: a plant extract common in muscle and bruise rubs
The bottom line? Judge the whole formula, not the CBD number alone.
Where a CBD Topical for Pain Works Best
A CBD topical for pain only reaches tissue near the skin. So where it hurts matters more than how many milligrams are in the jar.
Board-certified physician Leigh Vinocur makes the same point at GreenState: topicals suit localized joint, muscle and skin pain. Migraines, fibromyalgia and menstrual cramps are less likely to respond.
Check this chart before you buy:
| Where it hurts | Fit for a topical | Why |
|---|---|---|
| Hands, fingers, wrists | Good | Joints sit under thin skin; tested in the thumb arthritis trial |
| Jaw (TMJ) | Good | Tested in the 2019 jaw muscle trial |
| Knees and elbows | Good | Shallow joints, easy to cover (the one knee trial, after surgery, found no benefit) |
| Feet with burning or tingling | Possible | Tested in the 2020 nerve pain trial; see a doctor about numbness |
| Neck, shoulder and low back muscles | Possible | Muscle sits under the skin, and a cream covers large areas |
| Hips | Weak | The joint sits deep under thick muscle |
| Whole-body pain, migraine, cramps | Poor | The pain isn't in one spot near the skin |
Nerve Pain: Can a CBD Balm Help?
Maybe, with realistic expectations. In the Xu trial, CBD oil on the feet eased sharp, cold and itchy sensations in people with nerve damage. It was small, but it's the best evidence we have.
A CBD balm for nerve pain makes sense for feet and hands, since balms are concentrated and stay where you put them. No study shows CBD repairs nerves, though. See a doctor first if you notice:
- Numbness or weakness
- Pain that spreads or keeps getting worse
- Foot sores that are slow to heal
CBD Cream vs. Lotion vs. Balm: Which Format Fits?
The CBD inside matters less than you'd think. The base decides how a topical feels, how far it spreads and where it works best.
Searching for a CBD lotion for pain? Lotions are just thinner creams with more water. They suit big areas and fast application.
| Format | What it's made of | How it feels | Best for | HempLucid option |
|---|---|---|---|---|
| Cream | Water plus butters and oils | Absorbs and spreads easily | Large areas, daily use | CBDA + CBD Body Cream |
| Lotion | More water, thinner | Light, often pumpable | Large areas, quick use | None; the cream covers this job |
| Balm or salve | Oils and beeswax, no water | Thick, waxy, concentrated | Knuckles, elbows, jaw, feet | Cedarwood Body Balm or Lavender Body Balm |
| Stick or roll-on | Solid balm or liquid in an applicator | No mess on your hands | On the go | None |
Quick rule: cream for big areas, balm for small ones. A balm also stays put on a jaw or knuckle instead of sliding off.
How to Choose a CBD Pain Relief Cream: 6 Checks
Whether you call it a CBD cream for pain relief or a CBD pain relief cream, the label tricks are the same. The big number on the jar can mislead you.
Run these six checks before you buy. They take about a minute.
- A COA for your batch. A certificate of analysis from an independent lab should list potency, total THC in mg, pesticides, heavy metals, solvents and microbes. Why bother? A 2017 JAMA study found only about 31% of 84 CBD products bought online were accurately labeled. See how we test every batch, or look up your lot with the QR code on your label.
- Strength per gram, not per jar. Divide the mg of CBD by the grams of product. 1,000 mg in a 2 oz (57 g) jar is about 17.6 mg per gram. The same 1,000 mg in a 4 oz jar is about 8.8 mg per gram, half as strong. Then compare price per mg.
- Spectrum and THC. Full-spectrum keeps trace THC and other plant compounds; broad-spectrum and isolate don't. Compare full-spectrum vs. broad-spectrum vs. isolate and read up on the entourage effect. Drug tested? Read the safety section below first.
- Supporting ingredients. Menthol and camphor cool. Capsaicin warms. Sensitive skin? Patch test any new formula.
- Cosmetic or OTC drug label. A Drug Facts panel means an OTC pain reliever, with an active like menthol, methyl salicylate or lidocaine. The FDA says CBD doesn't belong in OTC drugs at all, and it warned two companies over CBD pain relievers in March 2021 (Hyman, Phelps & McNamara summary). So a CBD pain cream with a Drug Facts panel is a red flag, not a selling point.
- Base, allergens and red-flag claims. Watch for coconut oil, beeswax (not vegan) and fragrance. Skip any brand that claims its cream treats a disease; the Arthritis Foundation agrees.
Pass all six? Then you've found a cream worth testing on your own sore spot.
How to Use CBD Cream for Pain
Technique matters more than most people think. A good cream used badly is money down the drain.
Follow these steps with any cream or balm:
- Patch test a small amount on your inner arm and wait 24 hours.
- Apply to clean, dry skin. Never use it on broken or irritated skin.
- Use a pea-size amount for a knuckle or jaw and about a nickel-size amount for a knee or shoulder. Massage for 1 to 2 minutes until it soaks in.
- Wash your hands afterward, especially with menthol, and keep it away from your eyes.
- Give it 15 to 30 minutes (how long CBD takes to kick in), then reapply as the label allows.
- Keep a short pain log for 1 to 2 weeks before you judge it. The Arthritis Foundation recommends a symptom diary for any CBD trial.
One safety note: don't put a heating pad, hot water bottle or tight wrap over a menthol or capsaicin product. The FDA warned in 2012 that heat and tight bandages over these rubs have led to rare but serious burns.

Safety, Drug Tests and the December 2026 Hemp Rule
CBD creams are low risk for most adults. Low risk isn't zero risk, though.
Here's what to watch, as of November 2026.
Side Effects
Skin irritation is the main one. In a small 2026 balm trial, 24% of 21 women using CBD or THC balms on their hands reported minor irritation.
The FDA's bigger warnings, like liver injury, come mainly from an oral CBD drug. Stop using any cream that irritates your skin.
Medications
Talk to your doctor before using CBD if you take other medicines. The Arthritis Foundation flags these in particular:
- Blood thinners such as warfarin
- Corticosteroids such as prednisone
- Gabapentin or pregabalin
- Certain antidepressants, such as sertraline or fluoxetine
This interaction review covers the rest. Skip CBD during pregnancy and breastfeeding, since the FDA lists its effects on a developing baby as unknown. And CBD is no substitute for prescribed disease-modifying treatment.
Drug Tests
Every HempLucid topical is full-spectrum, so each one carries trace THC. A positive test from a cream is unlikely, but not impossible.
If you're tested at work, talk to your doctor first and consider THC-free CBD instead.
What Changes on December 11, 2026
A federal law signed in November 2025 redefines hemp. A stopgap funding bill (H.R. 6500), signed September 2, 2026, moved most of it from November 12 to December 11, 2026, according to Hemp Law Group. The key points:
- Hemp is measured by total THC, so THCA counts
- Finished products may hold no more than 0.4 mg of total THC per container
- Bills to delay the change to 2028 (H.R. 7024 and S. 3686) were still in committee as of October 19, 2026
Full-spectrum topicals can be affected, and some states set their own rules. Every HempLucid COA lists THC in mg per lot. See the HempLucid CBD FAQ for more on legal status.
HempLucid's CBD Cream and Balms
HempLucid makes three full-spectrum topicals. An independent lab tests every lot, and the LucidID QR code on each label links to that lot's COA.
Here's how they compare (prices as of November 4, 2026):
| Product | Cannabinoids per container | Notable ingredients | Format | Price |
|---|---|---|---|---|
| Full-Spectrum CBDA + CBD Body Cream | 1,000 mg (500 mg CBDA + 500 mg CBD) | Organic cocoa butter, coconut and olive oils, arnica, chamomile, aloe, blood orange oil; no menthol | Cream for larger areas | $69.95 |
| Cedarwood Body Balm | 250 mg CBD | Organic beeswax, coconut oil and cocoa butter, organic menthol, arnica, cedarwood and peppermint oils | Balm for small spots | $19.95 |
| Lavender Body Balm | 250 mg CBD | Organic beeswax, coconut oil and cocoa butter, organic menthol, arnica, lavender and peppermint oils | Balm for small spots | $19.95 |
The cream holds 4.83 out of 5 stars from 217 reviews, and customers talk a lot about how it feels:
"It goes on smoothly, leaves your skin soft, and has a pleasant scent." (Andrea O.)
What's CBDA? It's the raw, acidic form of CBD found in the living hemp plant. Research on it is mostly lab-based so far, so read What Is CBDA? for what's known.

Frequently Asked Questions
Does CBD cream really work for pain?
It can, for some people and some spots. Small trials of CBD cream for pain found benefits for nerve pain in the feet, jaw muscle pain and thumb arthritis.
Larger, longer studies are still missing, and an 80-person knee surgery trial found no benefit. Treat it as a low-risk experiment, not a sure thing.
How long does CBD cream take to work?
Topicals usually take 15 to 30 minutes. Menthol's cooling feel kicks in within seconds, which is a separate effect.
For a fair test, use it consistently for 1 to 2 weeks. Read more on how long CBD takes to kick in.
Is CBD cream or CBD oil better for pain?
They do different jobs. A cream works on one spot near the skin. Oil you swallow works through your whole body, more slowly.
Some people use both. For oral options, read our guide to CBD oil for pain, and ask your doctor about your medications.
Can I use a CBD balm for nerve pain?
You can try a CBD balm for nerve pain on your feet or hands. The 2020 Xu trial found topical CBD eased sharp and cold sensations from nerve damage.
There's no evidence CBD repairs nerves, though. Numbness, weakness or spreading pain needs a doctor's visit before any topical.
Will CBD cream show up on a drug test?
It's unlikely, but not impossible with a full-spectrum cream, which carries trace THC.
If you're tested, talk to your doctor and consider THC-free CBD instead.
Is CBD cream legal after December 11, 2026?
The new federal rule doesn't ban CBD by name. But as of November 2026, hemp products must stay at or under 0.4 mg of total THC per container from December 11, and some states set their own limits.
Check each product's COA for its THC in mg. The HempLucid CBD FAQ covers more legal questions.
The Bottom Line
CBD cream for pain is worth a try when your pain sits in one spot close to the skin. The evidence is small and mixed, so go in with fair expectations.
Before you buy, check three things:
- The COA for your batch
- The strength per gram
- The label: cosmetic or Drug Facts
Then give it two honest weeks with a pain log. Ready to start? Shop HempLucid's CBD topicals, or compare oral options in our CBD oil for pain guide.
